As the data controller, our clinic processes your personal data (such as identity, contact details and health information you provide in this form) in accordance with the Law on Protection of Personal Data No. 6698 (“KVKK”) for the purposes of evaluating your request, planning and carrying out your appointment and related health services, and contacting you when necessary.

Your data may be shared with our physicians, staff and business partners providing IT, call center or similar support services, limited to the purposes stated above and in compliance with KVKK.

You may apply to us at any time to exercise your rights under Article 11 of KVKK, including learning whether your data is processed, requesting information, correction, deletion and objecting to processing.
